What is MyChart?

MyChart is a free, convenient and secure online health management tool that allows you to email your doctor, get lab and test results, book an appointment, request prescription refills, view and pay your bill, and view your medical information. It is available 24 hours a day, 7 days a week on a desktop computer, a laptop, a tablet or a smartphone.

How do I get a MyChart account?

During your office visit, ask the staff to give you an activation code. This code is good for 14 days. Visit, click on the "Sign Up Now" link, enter your activation code and follow the instructions.

What computer hardware/software do I need to use MyChart?

MyChart requires only a connection to the internet and a Web browser. MyChart supports:

  • Internet Explorer 9.0 (standards mode and IE8 compatibility mode)
  • Internet Explorer 8.0 (standards mode only)
  • Mozilla Firefox 13.x and above
  • Safari 5.x on Mac/Windows
  • Google Chrome 20.x and above
  • Mobile App is available for all Smartphones

How do I get the MyChart mobile app on my smartphone or tablet?

First, activate your account by following the steps listed above. Then visit the Apple App Store (on Apple devices) or Google Play (on Android devices), type the word MyChart into the search box and download and install the app. Open the app and click New York. Scroll down and select Montefiore MyChart. Enter your login information.

How do I send a message to my doctor using MyChart?

Log into your account and click "Messaging" on the toolbar at the top of the page.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive a reply within 48 business hours. MyChart should not be used for urgent situations. Call your doctor's office if you require immediate attention, or dial 911 if you have an emergency.

How will I know when the doctor has replied?

An email will be sent to your personal email account letting you know that you have a message in your MyChart account. Log into MyChart to read the message.

Is MyChart secure?

MyChart protects the privacy and confidentiality of your personal health information and messages to and from your doctor. Only you and your doctor's authorized staff can read your MyChart messages.

Can I view a family member's health record in MyChart?

Parents (or legal guardians) can log into their own MyChart account and connect to their children's information. Information about an adult may be accessed by family members with written permission from the individual. Ask your doctor's office for a MyChart Proxy Consent Form. If the person is incapacitated, guardians, individuals with a power of attorney or other legally authorized representatives can be allowed to access the individual's information if they complete a MyChart Proxy Consent Form.

Can my spouse and I share one MyChart account?

No. Each adult must establish his or her own MyChart account.

My activation code does not work, what should I do?

Your activation code expires 14 days after it is given to you and is no longer valid after the first time you use it. If you have problems activating your account within those 14 days, please contact the MyChart Support Line at 1-855-226-3070; available 24 hours a day, 7 days a week for assistance.
